Description

The Nephrology Clinician provides high quality nephrology care to adult patients on a scheduled and unscheduled basis, consistent with accepted standards of clinical practice, including preventive activities such as screenings, examinations, comprehensive health assessments, diagnosis, treatment and follow-up of minor, acute and chronic nephrology conditions, with referrals and follow as indicated.

Essential Functions and Duties:

  1. Provides high-quality nephrology care which includes, screening, history taking and examination of adult patients; formulates diagnosis and institutes treatment based on objective data gathered, and makes appropriate referrals as indicated (i.e. diagnostic and specialty referrals), as indicated.

  2. Performs all procedures and applicable duties according to his/her scope and approved privileges.

  3. Works with other members of the medical team to ensure that high quality care is performed and coordinated to ensure clinical compliance with BMS policies, Federal, State, Joint Commission and Manage Care guidelines.

  4. Ensure that clinical documentation is completed in a timely manner in compliance with BMS policy, which includes the closing of encounters within 48 hours and the submission to billing as well as timely sign off on lab results and reports within 48 hours of receipt.

  5. Works with the other members of the medical team to promote and maintain high- quality medical care for all patients and coordinates patient care with other clinicians / specialists.

  6. Participates in regular chart reviews (including PEER Review) and other activities consistent with his/her role as a primary care clinician within the clinical practice.

  7. Complies with standard of care from American Association of Nephrology and other Accredited Nephrology Clinical Practice Guidelines.

  8. Participates in departmental meetings, clinical staff meetings, general staff meetings or any other meetings as set by the department, as indicated.

  9. Participate in all quality improvement activities, as indicated.

  10. Maintain compliance with all credentialing requirements.

  11. Keeps patient and doctor's relationship confidential, in compliance with HIPAA regulations.

  12. Abides by the Oath of Hippocrates in rendering health care to patients and the community.

  13. Will achieve or exceed the projected access goals.

  14. Timely completion of necessary continuing education.

  15. Other duties as assigned.

Requirements

Required Skills/Abilities:

  1. Thorough understanding of facilities planning principles, best practices, and procedures.

  2. Thorough understanding of local, state, and federal building codes, ordinances, and regulations.

  3. Strong communication skills.

  4. Ability to listen and respond appropriately to information received.

  5. Ability to prioritize and delegate multiple tasks to ensure the required documentation is completed correctly and in a timely manner.

Education and Experience:

  1. Board Certified or Board Eligible in Nephrology.

  2. Maintain a Current Licensed to practice as a Nephrology Clinician in New York State.

  3. Completed accredited training in an institution approved for post-graduate training in

Medicine and Nephrology.

  1. Previous clinical experience, preferably in Community Health Center setting.

  2. Computer literacy / Proficiency required.

  3. Bilingual a plus.

Physical Requirements:

  1. Prolonged periods sitting at a desk and working on a computer.

  2. Must be able to lift 25 pounds at times.

  3. Must be able to traverse all facilities in all types of weather.

Competency and Credentialing Requirements:

· Completion of annual education modules

· Annual Health Assessment

· Mandatory requirements as indicated by BMS.
