Essential Duties and Responsibilities

• Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
• Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
• Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
• Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
• Participate in care initiation for chronic care management services.
• Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
• Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
• Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system.

Requirements

• Master’s degree in nursing and graduate of an accredited Nurse Practitioner program.
• Unrestricted Nurse Practitioner license in the relevant state.
• Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed during the practice.
• Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.

Preferred Experience

• A minimum of 3 years of clinical experience, preferably in long-term care.
• Experience with eClinicalWorks (eCW) electronic health records.
• Must be completed 3600 clinical hours

Schedule

• Full and part-time opportunities available.
• On-site required.
