Job Description

Horizons is seeking a full time Day Program Coordinator at our Day Program site in Craig. The Day Program Coordinator facilitates day services for individuals with disabilities in the HCBS-DD and HCBS-SLS waivers, as well as private pay and state funded services. Responsible for oversight, program development and management, utilization review, quality assurance, and compliance with all relevant regulations while emphasizing person centered services, community integration, and utilizing natural supports. The Day Program Coordinator recruits, trains, supervises, and schedules Vocational Specialist(s) and Direct Support Professionals in alignment with the program budget. This position works closely with the Leadership Team, agency nurses, Residential Coordinator, Case Management Agency (CMA), families, and Direct Support Professionals (DSPs) to maintain high-quality, person centered services. Education and Experience Requirements: 1. Bachelors degree with related course work in management and human services or commensurate experience. 2. Working understanding of the IDD system, Medicaid HCBS-Waivers, and associated rules and regulations. 3. Prefer three (3) years experience working with budgets, program implementation, quality assurance, supervising staff, and person-centered thinking practices. Compensation Our generous benefit package includes insurance (medical, dental, vision, and life), PTO, paid holidays, and bereavement leave.
