Summary:

The Cardiology Nurse Navigator demonstrates critical thinking and uses the nursing process to assess and meet the need of cardiology patients by providing care coordination through the cardiology disease-specific continuum. This position acts as a liaison between patients, families, and caregivers and the healthcare delivery system to optimize patient outcomes while reducing barriers to care.

Responsibilities:

  1. Coordinates designated administrative operations for the department, under the general direction of the manager. Supports and upholds departmental policies and standards regarding patient flow and operations by working with departmental staff to maintain throughput processes.
  2. Coordinates Multidisciplinary Program.
  3. Coordination of care.
  4. Communication: Provides psychosocial support to and facilitates appropriate referrals for patients, families, and caregivers, especially during period of high emotional stress and anxiety. Builds therapeutic and trusting relationships with patients, families, and caregivers through effective communication and listening skills. Facilitates communication among members of the multidisciplinary cardiology care team to prevent fragmented or delayed care that could adversely affect patient outcomes.
  5. Participates in Development of Standards.
  6. Participates in Community Outreach Activities.
  7. Displays leadership.
  8. Performs miscellaneous duties.

Other information:

Education Requirements:

Bachelor's degree in nursing.

Master's degree in nursing is preferred.

Licensure/Certification Requirements:

Current license to practice as a Registered Nurse in North Carolina.

BLS Certification

Professional Experience Requirements:

3 years' experience in healthcare/medical - Nurse: Registered Nurse in an acute care setting

Prior experience in working directly with cardiac patients is preferred.
